Based on how unkempt his TV tray station is (boxes, mug, and all), I wouldn't be surprised if Cliff's underwear were in fact ketchup-stained (as opposed to heart-speckled.)
He's trying to use that one, poor, flimsy TV tray as a coffee table or nightstand! LOL Poor thing. Just never can win...
